Case Manager Opportunity for suitably qualified Social Worker, Nurse, Occupational Therapist, Physiotherapist or Speech and Language Therapist

 

CCMS is an award winning case management service specialising in the rehabilitation of adults & children following brain / spinal injury.  As case managers, we assess rehabilitation needs, report to court, commission services and lead multi-disciplinary teams, providing rehabilitation and support in the community to assist clients to reach their optimum potential.

 

Ideally you will have already worked as a Case Manager, Social Worker, Nurse, Occupational Therapist, Physiotherapist or Speech and Language Therapist and have experience and skills in one of these disciplines or associated fields.

 

We are seeking dynamic individuals with the following:

 

  • Professionally qualified as a Case Manager, Social Worker, Nurse, Occupational Therapist, Physiotherapist or Speech and Language Therapist
  • Experience working as a Case Manager, Social Worker, Nurse, Occupational Therapist, Physiotherapist or Speech and Language Therapist
  • Structured and logical thinking style that is comfortable operating both at a detail level and at a directional level
  • Excellent report writing skills and effective verbal communication
  • Ability to build therapeutic relationships quickly and have strong interpersonal skills with clients and professionals
  • Ability to work autonomously and also as part of a team
  • Ability to demonstrate excellent time management skills and also co-ordinate others when working to deadlines
  • Ability to apply new learning to work situation
  • Ability to transition from a Social Worker, Nurse, Occupational Therapist, Physiotherapist or Speech and Language Therapist in to the role of Case Manager
  • Must be IT literate and have ability to learn new systems quickly.
  • Strong problem solving skills and the ability to manage difficult situations
  • Must have experience dealing with clients with Acquired Brain Injury / Spinal Injury and/or Cerebral Palsy.
  • Experience in neuro rehabilitation would also be an advantage.

Care assistant required for a 14-year-old boy living with his family in the Dewsbury area.  He has physical, sensory & cognitive disabilities & is wheelchair dependent following a serious accident. 

 

Applicants require empathetic & dynamic approach and the ability to promote and encourage therapies & engage in everyday home and community activities ensuring, wherever possible, that he is facilitated to engage in the activities of his choice and to be supported to maximise his independence and communication.  The care assistants will work with SALT, Physiotherapist and Occupational Therapist to support communication and Psychologist to support behaviour.

Home & community activities are essential. 

 

Flexible support is required for some after school shifts plus weekend hours. Extra shifts will be required during School holidays.

 

Personal care required, and driver essential.

The client lives independently in his own home and requires support with daily living, to attend medical appointments, to accompany to the gym and other social activities.

 

Applicants require empathetic & dynamic approach and the ability to promote and encourage daily routine and engage in home & community activities is essential.

 

Driver / full license and own vehicle preferred.

Successful applicants will receive support, training & supervision.

 

The successful candidate needs to be flexible as the rota could change due to do medical appointments or accompanying the client on holidays as and when required.

Brief description:

Care Assistants required for a happy 12 year old girl with additional needs who lives in the family home.  We are looking for people who have a genuine desire and commitment to providing the high level of care she is used to which enables her to maximise her life to the full.

The successful applicant will work as part of a team and be responsible for supporting her with all personal cares, feeding, medication giving and moving & handling.  You will provide support with taking her to hydrotherapy, carrying out physiotherapy and taking her out in the community which she really enjoys.

.

Applicants must be enthusiastic and have a sense of humour.  Full training both through CCMS and in the family home will be given to support you in carrying out this role.  Experience not essential.

.

Due to increased care requirements we are looking to recruit committed female care assistants for permanent part time posts to join an existing team:

Post 1, approx. 24hrs per week

Post 2, approx. 12hrs per week

Hours are over a 2 week rota

.

As you would be working as part of a team you would be required to cover some annual leave for the other team members and additional flexible working hours required during school holidays.

Driver with full UK driving license would be preferred.
